About The Position

Works as part of an interdisciplinary team to help older adults maintain and improve their mobility, strength, balance, and overall physical function so they can remain safe and independent in their homes and communities. They assess each participant’s physical abilities, develop individualized treatment plans, provide therapeutic exercises and mobility training, monitor progress, adjust care based on needs, and educate participants and caregivers on safe movement, fall prevention, and appropriate home strategies. The PT also collaborates with doctors, nurses, occupational therapists, and other team members to coordinate care and support quality of life for frail elderly participants enrolled in the PACE program.

Requirements

  • Licensed Physical Therapist (PT) in the state of Kansas
  • Graduation from an accredited physical therapy program
  • Experience working with older adults or geriatric populations
  • Knowledge of rehabilitation principles, mobility training, and fall prevention
  • Ability to perform physical assessments and develop individualized treatment plans
  • Strong communication and documentation skills
  • Ability to work effectively as part of an interdisciplinary healthcare team
  • CPR certification (or ability to obtain)

Nice To Haves

  • Experience in a PACE program, home health, long-term care, or skilled nursing setting
  • Familiarity with chronic disease management and age-related conditions
  • Experience providing caregiver education and patient training
  • Knowledge of adaptive equipment and assistive devices
  • Strong organizational and time-management skills
