About The Position

The Physical Therapist provides high-quality physical therapy services to older adults across home and center settings within a multidisciplinary care model. Working collaboratively with physicians, nurses, and social workers, the PT develops individualized treatment plans that improve mobility, function, and overall quality of life. This position combines clinical expertise with compassionate care to promote independence and well-being in every participant.

Requirements

  • Bachelor’s Degree in Physical Therapy required
  • Completion of an accredited Physical Therapy program recognized by the American Physical Therapy Association (APTA) or equivalent organization approved by CMS
  • Active California Physical Therapist (PT) license
  • Minimum 1 year of experience working with frail or elderly populations
  • Proof of medical clearance for communicable diseases and current immunizations

Nice To Haves

  • 2+ years of physical therapy experience
  • Master’s (MSPT) or Doctorate (DPT) in Physical Therapy
  • Bilingual in Spanish, Mandarin, or Cantonese
  • Experience with PACE, Adult Day Health Care (ADHC), or Community-Based Adult Services (CBAS) programs
  • Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

  • Perform physical therapy evaluations, treatments, and follow-up assessments for PACE participants
  • Develop individualized therapy plans addressing mobility, balance, strength, and functional goals
  • Collaborate with the Interdisciplinary Team (IDT) to support coordinated care planning and rehabilitation goals
  • Educate participants and caregivers on home exercise programs, durable medical equipment (DME), and safety practices
  • Conduct home visits as needed and document therapy services, participant progress, and outcomes
  • Contribute to quality assurance, infection control, and safety programs
  • Assist in ordering and maintaining DME inventory
  • Provide subject matter expertise for staff education, program development, and therapy standardization
  • Participate in staff meetings, clinical audits, and data collection initiatives
