About The Position

FOX Rehabilitation is expanding in South Lakewood, New Jersey, and is seeking passionate, licensed Physical Therapists to provide exceptional one-on-one care to older adults. This role involves delivering services directly in patients' homes or within Senior Living Communities, utilizing FOX's signature Geriatric House Calls™ model. The primary goal is to help patients enhance their mobility, independence, and overall quality of life. This opportunity offers significant autonomy and flexibility, allowing therapists to design and manage their own treatment schedules and cultivate strong, lasting patient relationships. Clinicians also have the option to integrate telehealth and other virtual care methods into their practice, which provides greater flexibility and ensures continuity of care. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Physical Therapy program
Basic computer and EMR proficiency
Current CPR certification

Responsibilities

Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
