About The Position

Meadowood Home Health is a compassionate, patient-centered home health agency dedicated to helping individuals regain independence and improve quality of life in the comfort of their own homes. We serve our residents and the community with a multidisciplinary team focused on personalized, high-quality care. Position Summary We are seeking a licensed Physical Therapist to join our team on a PRN basis. In this flexible role, you will provide in-home physical therapy services to patients with a variety of conditions, including orthopedic injuries, neurological disorders, post-surgical recovery, chronic pain, and mobility limitations. As a PRN therapist, you will enjoy autonomy in scheduling while contributing to meaningful patient outcomes.

Requirements

  • Current, active Physical Therapist license in PA
  • Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy preferred)
  • Minimum 1–2 years of clinical experience (home health experience strongly preferred but not required)
  • Strong assessment, clinical reasoning, and documentation skills
  • Excellent communication and interpersonal abilities; comfortable working independently in patients' homes
  • Reliable transportation, valid driver's license, and current auto insurance

Responsibilities

  • Perform comprehensive initial evaluations and ongoing assessments of patients' physical function, mobility, strength, range of motion, gait, balance, and home environment
  • Develop and implement individualized, physician-ordered treatment plans to relieve pain, restore function, prevent disability, and promote maximum independence
  • Administer therapeutic exercises, manual therapy, modalities, gait training, transfer training, and patient/family education
  • Instruct patients and caregivers in safe techniques, home exercise programs, use of adaptive equipment, and fall prevention strategies
  • Monitor patient progress, modify treatment plans as needed, and collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other care team members
  • Document all evaluations, treatments, progress notes, and discharge summaries accurately and timely in accordance with agency and regulatory standards (OASIS experience a plus)
  • Ensure compliance with state/federal regulations, agency policies, and best practices in home health care
