Requirements

Active Physical Therapy License (Arizona)
Valid Driver's license
Reliable transportation

Nice To Haves

You live in Phoenix or the East Valley (Chandler, Gilbert, Mesa, etc.)
You have prior home health experience

Responsibilities

Assess patients' physical abilities and develop individualized treatment plans.
Implement therapeutic interventions to improve mobility, strength and overall function.
Educate patients and families on exercises & techniques to support recovery and prevent injuries.
Monitor patient progress and adjust treatment plans to ensure optimal outcomes.
Collaborate with a multidisciplinary healthcare team to coordinate care.
Maintain accurate and timely documentation.
Coach and support caregivers.
