About The Position

Responsibilities: - Provide physical therapy services to patients, including evaluation, treatment planning, and implementation of treatment plans. - Develop and implement individualized care plans based on patient needs and goals. - Monitor and document patient progress, adjusting treatment plans as necessary. - Collaborate with other healthcare professionals to ensure comprehensive patient care. - Educate patients and their families on exercises and techniques for at-home care. - Stay up-to-date with advancements in physical therapy techniques and research.

Requirements

Valid license to practice as a Physical Therapist.
Knowledge of medical terminology, anatomy, and physiology.
Strong interpersonal and communication skills to effectively interact with patients and healthcare team members.
Ability to assess patient needs and develop appropriate treatment plans.
Compassionate and patient-centered approach to care.

Nice To Haves

Experience in Home Health is preferred.
