About The Position

We Are Inspired to Serve. Join us! The physical therapist is responsible for proving physical therapy services and for adherence to all conditions outlined in the personnel services agreement. ESSENTIAL RESPONSIBILITIES: Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care. Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM – potential for rehab. Directing physical therapy treatment. Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. May train patient in the use of prosthetic device. Participates in discharge planning for patient and in peer consultation process. Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed. When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.

Requirements

  • Bachelor’s Degree in Physical Therapy approved by an accredited organization.
  • PT License and CPR certification, if required by state
  • 2+ Years
  • Ability to show compassion and understanding.
  • Knowledge in HIPAA laws and confidentiality policies.
  • Ability to create and implement treatment plans based on patients’ specific condition and needs.
  • Ability to evaluate, conduct and development customized treatment plans based on findings.
  • Strong observation, time management, detail orientation, communication and interpersonal skills.
  • Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
  • Ability to operate various equipment base on department.
  • The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms.
  • Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
  • Clarity of Vision: For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
  • For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
  • For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Nice To Haves

  • Master’s Degree in Physical Therapy approved by an accredited organization.
