About The Position

As a Physical Therapist specializing in home health care, you will play a critical role in improving the mobility, function, and quality of life for patients recovering from injury, surgery, or managing chronic conditions. You will assess patients' physical capabilities and develop personalized rehabilitation plans that address orthopedic and sports-related injuries. Your expertise in manual therapy and orthopedic rehabilitation will be essential in delivering hands-on treatments that promote healing and reduce pain. Working primarily in patients' homes across the United States, you will provide compassionate, patient-centered care that supports independence and long-term wellness. Ultimately, your work will contribute to patients' successful recovery and their ability to return to daily activities with improved physical function.

Requirements

  • Successful completion of accredited physical therapy program and state licensure exam.
  • Valid Physical Therapy license
  • 1 year of home health experience preferred
  • CPR certification
  • Valid Driver’s license
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred
  • Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
  • Must be able to lift, carry, pull, or push up to 100 lbs.

Nice To Haves

  • 1 year of home health experience preferred
  • HomeCare HomeBase experience preferred

Responsibilities

  • Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
  • Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  • Applies concepts of infection control/universal precautions in performing patient care
  • Assure continuity of quality patient care delivered with appropriate documentation
  • Provide effective communication to patient/family, team members and other health care professionals
  • Promote agency philosophy and administrative policies
  • Maintain and complete all documentation within company stated guidelines.
