Requirements
- Applicants must hold an active Indiana license in their respective discipline (Physical Therapist) or be eligible to obtain Indiana licensure.
- Eligibility includes successful completion of an accredited educational program appropriate to the discipline, passing the required national and/or state licensure examination(s), and meeting all [state] licensure requirements.
