About The Position

Physical Therapist (PT) – Contract (HCS Waiver) We are seeking a Contract Physical Therapist (PT) to provide skilled therapy services under the Texas Home and Community-based Services (HCS) Medicaid Waiver Program. This position offers a flexible, PRN-based schedule and is ideal for clinicians looking to supplement their current caseload. Compensation: $72.43 per hour Location: Home and community settings (Texas) Schedule: Part-time / Contract (hours vary based on client needs) Position Overview The Physical Therapist will provide evaluation and treatment services to individuals with intellectual and developmental disabilities (IDD) enrolled in the HCS program. Services are delivered in home and community settings and focus on improving mobility, increasing independence, and enhancing overall physical functioning in accordance with HHSC and HCS waiver requirements.

Requirements

  • Graduate of an accredited Physical Therapy program (Master’s or Doctorate)
  • Current, active Physical Therapist (PT) license in Texas
  • At least 1 year of experience in IDD, home health, pediatrics, or geriatrics (preferred)
  • Strong knowledge of therapy practices and person-centered care
  • Excellent communication and documentation skills
  • Ability to work independently in home and community settings
  • Current CPR certification
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to manage a flexible and changing caseload
  • Must pass all required background and credentialing checks

Nice To Haves

  • Experience providing services within the Texas HCS or other Medicaid waiver programs
  • Familiarity with HHSC documentation and compliance standards

Responsibilities

  • Conduct physical therapy evaluations and develop individualized treatment plans
  • Provide skilled physical therapy services in home and community-based settings
  • Implement interventions to improve mobility, strength, coordination, and functional independence
  • Collaborate with interdisciplinary team (IDT) members, caregivers, and family members
  • Monitor client progress and modify treatment plans as needed
  • Maintain accurate, timely documentation in compliance with HHSC, Medicaid, and HCS guidelines
  • Participate in IDT meetings as required
