About The Position

Therapy Partner Solutions, a therapist-owned and operated company since 2016, is dedicated to providing exceptional therapy services. This role is for a Physical Therapist to join an Ortho-Based Outpatient in Home Transition Program in partnership with Luminis Health. This innovative care model focuses on short-term in-home rehabilitation, typically 2–3 visits, before transitioning patients to outpatient care. This position offers an alternative to traditional home health by eliminating Start of Care (SOC) visits, case conferences, and OASIS documentation, focusing strictly on an orthopedic caseload with evaluation and follow-up visits. The expectation is to see approximately 6–7 patients per day, with a goal of 30 visits per week.

Requirements

• Graduate (current or pending) from an accredited physical therapy program
• Active or pending state physical therapist license
• Comfort working independently in the home setting

Responsibilities
