About The Position

Therapy Partner Solutions, a therapist-owned and operated company since 2016, is dedicated to providing exceptional therapy services. They are the provider of outpatient therapy services for Luminis Health, working as part of a strong, multidisciplinary team that includes physical therapists, physicians, and other healthcare professionals to deliver coordinated, patient-centered care. Clinicians have the opportunity to participate in Luminis Health’s Orthopedic Residency Program for advanced training and mentorship. The company supports its clinicians through professional development opportunities and eligibility for the Public Service Loan Forgiveness (PSLF) Program. This role is for a Physical Therapist to join an Ortho-Based Outpatient in Home Transition Program in partnership with Luminis Health. This innovative care model focuses on short-term in-home rehabilitation, typically 2–3 visits, before transitioning patients to outpatient care. This position offers an alternative to traditional home health by eliminating Start of Care (SOC) visits, case conferences, and OASIS documentation, focusing strictly on an orthopedic caseload with evaluation and follow-up visits. The expectation is to see approximately 6–7 patients per day, with a goal of 30 visits per week. Therapy Partner Solutions aims to protect clinicians' time, support their clinical work, and help them succeed by offering thoughtfully managed caseloads, minimal drive time between visits, consistent clinical and administrative support, one-on-one patient care, and flexible scheduling to support work-life balance.

Requirements

  • Graduate (current or pending) from an accredited physical therapy program
  • Active or pending state physical therapist license
  • Comfort working independently in the home setting

Responsibilities

  • Focus strictly on evaluation and follow-up visits
  • See approximately 6–7 patients per day with a goal of 30 visits per week
