About The Position

TriHealth is actively recruiting for a Physical Therapist to join the PACE of Cincinnati PACE Center located in Norwood Ohio on an Optional basis. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Job Overview: The PACE Physical Therapist is responsible for the physical therapy program for PACE participants. The PACE Physical Therapist is responsible for evaluating, assessing, and treating PACE participants with musculoskeletal conditions, disabilities, or movement impairments by developing individualized care plans utilizing acute and maintenance physical therapy interventions. The PACE Physical Therapist is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing physical therapy assessments for new PACE participants and physical therapy reassessments on existing PACE participants as part of the PACE IDT care planning process. The PACE Physical Therapist is responsible for providing physical therapy education to participants, caregivers, and PACE staff to address mobility and functional issues.

Requirements

  • Master's Degree in Physical Therapy, if obtained prior to the year 2000. (Required)
  • Doctoral Degree in Physical Therapy, if obtained after the year 2000. (Required)
  • Basic Life Support (BLS) Required
  • Physical Therapist Required

Nice To Haves

  • 1 - 2 years of experience as a Physical Therapist. (Preferred)

Responsibilities

  • Participates as a PACE Interdisciplinary Team (IDT) member and is responsible for completing physical therapy assessments for new PACE participants and physical therapy reassessments on existing PACE participants as part of the PACE IDT care planning process.
  • In collaboration with the primary care provider and IDT, develops and implements physical therapy care plans for participants, identifying and considering the medical, physical, social, and emotional needs of PACE participants.
  • Attends IDT meetings and morning meetings and reports on participants' mobility status, including changes to individual participant functional ability.
  • Based on clinical observations or physical therapy reassessments, makes recommendations to the IDT for incorporating or modifying physical therapy interventions to participant care plans.
  • Monitors PACE participants for changes in their health or psychosocial status requiring a physical therapy reassessment. Promptly communicates pertinent changes in participant status to the IDT.
  • Responsible for providing physical therapy education to participants, caregivers, and PACE staff to address mobility and functional issues. Home visits may be necessary to provide participants and caregivers with physical therapy education, durable medical equipment training and education, or physical therapy assessments/reassessments.
  • Serves as a physical therapy subject matter expert and communicates best practices for a frail elderly population to the IDT, participants, and caregivers. Supports the PACE QI program and participants in PACE QI activities as assigned.
