Position Summary: Assesses and restores patients' function, alleviates pain, and prevents disabilities by designing, planning, and implementing medically prescribed physical therapy interventions. Requirements
- Master’s Degree in Physical Therapy or Bachelor’s degree with prior experience as a licensed Physical Therapist required.
- Required Licensure/Certification: Current New York State license or a limited permit as a Registered Physical Therapist. If a New York State license or limited permit is not held, the applicant must be hired within 90 days of graduation from a New York State or APTA-accredited physical therapist education program and meet the Practice Exemption under New York State Physical Therapy License Requirements. If working in Homecare or traveling between locations, a NYS license is required.

Responsibilities
- Treatment & Planning. Meet patient goals and needs by providing quality, evidence-based care; assess, develop and implement an individualized treatment plan; help the patient accomplish treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage and assist patients in performing physical activities, such as non-manual exercises, ambulatory functional activities and daily-living activities
- Discharge Management. Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs
- Leadership. Develop physical therapy staff by providing information; develop and conduct in-service training programs
