We are strengthening our Santa Clara County team and are seeking a Physical Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them. Requirements
- Current California Physical Therapist license
- Home health experience preferred; strong outpatient, inpatient, or rehab PTs encouraged to apply
- Valid California driver’s license
- Strong clinical judgment, organization, and time-management skills

Responsibilities
- Independently perform physical therapy evaluations and reassessments
- Develop and manage individualized plans of care
- Deliver skilled therapy focused on mobility, function, fall prevention, and independence
- Supervise and collaborate with PTAs as applicable
- Communicate directly with physicians and the interdisciplinary team
- Educate patients and caregivers
- Complete documentation in Kinnser supported by AI-assisted charting tools
- Serve as the primary PT voice in patient care planning
