About The Position

Human Touch Home Health is seeking an experienced Physical Therapist to provide in-home therapy services in Spotsylvania County.

Requirements

• Licensed Physical Therapist – Virginia
• Minimum 3 years PT experience (home health preferred)
• EMR experience (WellSky/Kinnser or Homecare Homebase preferred)
• OASIS experience a plus
• Valid driver’s license & reliable transportation

Responsibilities

• Perform in-home PT evaluations
• Develop and implement treatment plans per physician orders
• Provide skilled therapy to improve mobility, strength, and balance
• Educate patients and caregivers
• Complete timely documentation and collaborate with care teams
• Supervise PTA as applicable
