This role offers autonomy, flexibility, and strong interdisciplinary support.
Requirements
- Graduate of accredited PT program
- Active Virginia PT license
- Minimum 3 years clinical experience (home health preferred)
- Ability to supervise PTAs
- Reliable transportation & valid driver’s license
Nice To Haves
- OASIS experience preferred
- EMR experience (WellSky/Kinnser/HCHB a plus)
Responsibilities
- Perform comprehensive patient assessments
- Develop and implement individualized care plans
- Provide skilled PT to improve mobility, strength, and function
- Educate patients/caregivers on exercises and safety
- Document visits accurately in EMR
- Collaborate with physicians and care team
- Ensure compliance with Medicare & HIPAA standards
