Requirements

  • A current and valid California Physical Therapist (PT) License.
  • A compassionate, "patient-first" mindset with a focus on functional outcomes.
  • Excellent communication skills and the ability to work independently within a team structure.

Responsibilities

  • Lead the Path to Recovery: Develop evidence-based treatment plans and secure physician approvals to help residents regain mobility and independence.
  • Clinical Excellence: Treat patients according to personalized care plans, ensuring the highest standards of therapeutic intervention.
  • Mentorship: Supervise Physical Therapy Assistants (PTAs) and assist the nursing department in training Restorative Aides.
  • Integrated Care: Play a key role in Patient Care Conferences, Utilization Reviews, and Rehabilitation Conferences to ensure holistic resident care.
  • Precision Documentation: Maintain meticulous records, including evaluations and progress notes, in full compliance with the PT Board and third-party payer requirements.
  • Empowerment: Facilitate discharge planning by providing caregiver training and recommending essential durable medical equipment (walkers, braces, grab bars) for a safe transition home.
  • Relationship Building: Foster positive, professional relationships with residents, their families, and your clinical colleagues.
