Check out our website: Home - Fortis Job Summary We are seeking a compassionate Part-Time/PRN Physical Therapist to join our team. In this role, you will provide skilled therapy in patients’ homes, helping them achieve their rehabilitation goals and regain independence. You will work closely with patients and caregivers to develop and implement individualized care plans that support function, safety, and overall quality of life.

Requirements

  • Graduate of an accredited physical therapy program (APTA or AMA approved) (required).
  • Licensed as a Physical Therapist in the state of Indiana (required).
  • Minimum of 1 year of experience as a Physical Therapist (required)

Responsibilities

  • Provide skilled physical therapy in patients’ homes according to the physician’s plan of care, seeing multiple patients per day.
  • Evaluate and assess patient function and assist in developing and updating individualized care plans.
  • Deliver treatments to relieve pain, restore function, and support maximum performance using appropriate modalities.
  • Monitor patient progress, document outcomes, and communicate changes to the physician, DON, and care team.
  • Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive devices or equipment.
  • Reassess patient needs regularly and revise plans of care as appropriate.
  • Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
  • Participate in quality improvement initiatives, staff meetings, and ongoing education.
  • Provide required supervision of the Physical Therapy Assistant at least every 30 days.
