About Twin Falls Transitional Care Center Twin Falls Transitional Care provides patient-centered rehabilitation and 24-hour skilled nursing care , with a focus on individualized treatment plans that enhance each patient's independence and quality of life. Requirements

• Current Idaho PT license in good standing
• Minimum Bachelor's degree from an accredited college or university
• Organized, self-starter who enjoys working with the geriatric population
• Ability to supervise a PTA

Nice To Haves

• At least one year of experience in a skilled nursing facility.
• If you are a caring, passionate, licensed therapist who loves working with older adults, we would still love to hear from you.

Responsibilities

• Evaluating patients and developing individualized treatment plans
• Occasionally treating clinically complex patients
• Performing and progressing gait training and therapeutic exercise programs
• Providing balance training and fall-prevention interventions
• Completing wheelchair and equipment assessments
• Performing and assisting with all types of transfers
• Educating patients and families on safety and mobility strategies
• Supervising a PTA
• Collaborating closely with nursing and the interdisciplinary care team
