About The Position

We are seeking a licensed Physical Therapist (PT) to provide high-quality rehabilitative care to patients in a post-acute or skilled nursing setting. The Physical Therapist will evaluate patient needs, develop individualized treatment plans, and implement therapeutic interventions to improve mobility, strength, balance, and overall functional independence.

Requirements

• Active Physical Therapist (PT) license in the practicing state
• Degree in Physical Therapy from an accredited program
• Experience in skilled nursing, rehabilitation, or post-acute care preferred
• Strong clinical, documentation, and communication skills
• Ability to work independently and as part of an interdisciplinary team
• Compliance with onboarding, credentialing, and facility requirements is required

Nice To Haves

• Previous contract or travel therapy experience
• Familiarity with electronic medical record (EMR) systems

Responsibilities

• Evaluate patients’ physical therapy needs and establish individualized treatment plans
• Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
• Document evaluations, treatment sessions, and progress notes accurately and timely
• Collaborate with interdisciplinary care teams to coordinate patient-centered care
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Ensure compliance with state regulations, facility policies, and professional standards
• Maintain productivity expectations while delivering quality patient care
