About The Position

We are seeking a skilled and motivated Outpatient Physical Therapist (PT) for a 13-week contract assignment in Woodsville, NH. The Physical Therapist will evaluate, plan, and implement individualized treatment programs to help patients improve mobility, reduce pain, and restore functional independence in an outpatient setting.

Requirements

• Minimum Bachelor’s degree in Physical Therapy
• Active New Hampshire Physical Therapist (PT) license
• BCLS certification (required)
• Minimum 2+ years of Physical Therapy experience
• Specialty experience in Physical Therapy (required)

Responsibilities

• Evaluate patients’ physical conditions, functional limitations, and rehabilitation needs
• Develop and implement individualized treatment plans based on patient assessments and physician referrals
• Provide therapeutic exercises, manual therapy, and modalities as appropriate
• Monitor patient progress and modify treatment plans to ensure optimal outcomes
• Educate patients and families on injury prevention, home exercise programs, and wellness strategies
• Accurately document evaluations, treatment sessions, and patient progress in accordance with facility and regulatory standards
• Collaborate with physicians and multidisciplinary healthcare teams to coordinate patient care
• Maintain compliance with professional standards, safety guidelines, and infection control practices
