About The Position

Physical Therapist (PT) 13 Week Contract Temp to Hire We’re seeking a dynamic Physical Therapist for a 13 week contract possible temp to perm who’s ready to hit the ground running. This role spans home health visits, interdisciplinary team (IDT) meetings, and in-clinic care—offering a unique blend of autonomy and variety. As a Physical Therapist, here’s a big-picture view of what you’ll do: 13 week contract possible temp to hire. Clinic Coverage and In Home visits/assessments IDT participation. Conduct in-person assessments to evaluate participants’ functional abilities, home environment, and rehabilitation needs. Recommend and coordinate adaptive equipment, home modifications, and therapy services to promote safety and independence. Develop, implement, and revise individualized treatment plans based on participant progress and IDT collaboration. Deliver PT interventions across settings including the clinic, participants’ homes, and contracted facilities. Educate participants and caregivers on equipment use, home safety strategies, and ADL programs. Maintain timely, accurate documentation of assessments, treatment, and participant progress per regulatory and organizational standards. Actively participate in daily IDT meetings, contributing to comprehensive care planning and 24-hour care coordination. Supervise Physical Therapy Assistants (PTAs), including review of care plans, co-signing documentation, and providing ongoing mentorship. Train staff in safe PT practices and equipment use to enhance participant care. Support quality improvement, utilization review, and compliance initiatives. Perform other IDT duties as needed within scope of practice and competencies. Maintain current licensure, certifications, and participation in ongoing professional development. Maintain appropriate access and confidentiality regarding Personal Health Information (PHI), in accordance with organizational and departmental guidelines.

Requirements

  • Graduate with a degree in an PT program from a university, which meets, the educational and field work requirements of the American Physical Therapy Association.
  • Successful completion of the national examination for PTs and SC license required.
  • 3 years PT experience in various settings preferred
  • Experience with geriatric/frail population
  • Looking for a long-term commitment
  • Availability for at least three 8-hour days Monday-Friday, preferred
  • Strong communication skills with the ability to collaborate effectively across all levels of clinical and non-clinical staff.
  • Proven ability to multitask and prioritize responsibilities in a dynamic healthcare environment.
  • Competent in reading and interpreting technical instructions related to patient care plans and procedures.
  • Skilled in observing and assessing patients through both visual and auditory cues.
  • Adaptable and resilient in managing frequent changes in patient needs and care settings.
  • Must provide a valid driver’s license and auto insurance
  • Current CPR/BLS certification required
  • Ability to pass drug screening and backgrounds
  • Up to date required immunizations (TDP, MMR, Varicella, and Flu) statement of good health
  • Must meet all applicable health and physical requirements necessary to safely provide patient care
