About The Position
The Physical Therapist PRN plays a critical role in delivering high-quality rehabilitative care within a Skilled Nursing Facility setting. This position focuses on assessing, planning, and implementing individualized physical therapy treatment plans to improve patients' mobility, strength, and overall functional independence. The therapist will collaborate closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care. The role requires flexibility and adaptability to meet the varying needs of patients on a PRN (pro re nata, or as-needed) basis, ensuring timely and effective therapy interventions. Ultimately, the Physical Therapist PRN contributes to enhancing patients' quality of life and facilitating their safe return to daily activities or home environments.
Requirements
- Successful completion of accredited physical therapy program and state licensure exam.
- Valid Physical Therapy license
- 1 year of home health experience preferred
- CPR certification
- Valid Driver’s license
- Valid Auto Insurance
- HomeCare HomeBase experience preferred

Responsibilities
- Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
- Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Applies concepts of infection control/universal precautions in performing patient care
- Assure continuity of quality patient care delivered with appropriate documentation
- Provide effective communication to patient/family, team members and other health care professionals
- Promote agency philosophy and administrative policies
- Maintain and complete all documentation within company stated guidelines.
