About The Position

As a valued member of our interdisciplinary rehabilitation team at Alta Bates Summit Medical Center, you’ll play a vital role in helping patients regain independence and improve quality of life. You’ll work with individuals recovering from stroke, brain and spinal cord injuries, complex orthopedic procedures, and other medically challenging conditions that impact daily function. In this per diem role, you’ll assess patient needs and design personalized treatment plans that restore essential life skills—from self-care and mobility to vocational and homemaking abilities. Through compassionate care, skilled observation, and clear communication, you’ll build trust with patients, families, and fellow healthcare professionals. You’ll ensure compliance with all regulatory standards while maintaining a safe, respectful environment. Additionally, you’ll provide clinical supervision to therapy assistants and aides, and may serve as a mentor to students during clinical rotations. Requirements

Graduate of an accredited physical therapy program
OR graduate of an accredited physical therapy program that can be demonstrated with a valid PT license
PT-Physical Therapist
OR Will also consider approved license applicant or resident training program employee.
BLS-Basic Life Support Healthcare Provider
Department: Home Health, DL-Valid Drivers License Class C
Department: Home Health, AUTO-Automobile Insurance
2 years of recent relevant experience.
Critical thinking, complex problems solving, decisive judgement and ability to work independently.
Knowledge and application of professional practice and regulatory requirements.
Must be able to work in demanding work environment, organize multiple priorities completing work duties within expected timelines and requirements.
Computer and required technology proficiency/competencies.
Demonstrates effective and efficient professional communication (verbal & written) skills as well as interpersonal skills, conflict resolution, teamwork/collaboration, customer service and community relations abilities.
Able to retain and apply new knowledge & skills.
Keeps abreast of professional practice development and advancement.
