Physical Therapist - Home Health (South)
About The Position
We are hiring Physical Therapists for Full-time, Part-time or Per-Diem work schedules. As a trusted community-based, nonprofit health agency, RVNAhealth provides high-quality care that supports patients where it matters most: at home . We are currently seeking a skilled and compassionate Physical Therapists to join our team and provide in-home therapy services across Southern Fairfield County , including Norwalk, Stamford, Darien, Greenwich and surrounding towns. What You'll Do As a Home Health Physical Therapist , you will: Perform comprehensive physical therapy evaluations and collaborate with physicians, nurses, patients, and families to develop individualized care plans. Conduct OASIS assessments Deliver goal-oriented, evidence-based therapy in the home setting to help patients regain function and independence. Provide education and instruction to patients, families, and caregivers. Identify and refer for additional services or next level of care as needed.
Requirements
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Connecticut PT license in good standing
- 2+ years of clinical experience (home health experience preferred)
- Proficiency in OASIS documentation and EMR systems (tablet provided)
- Strong understanding of therapy best practices in home-based settings
- Excellent communication, time management, and problem-solving skills
- Valid driver's license and reliable vehicle for travel throughout Fairfield County
