PT or PTA - Physical Therapist Assistant (Per Visit)
About The Position
Abcor Home Health is looking for a dedicated Physical Therapist Assistant (PTA) or Physical Therapist (PT) to join our team in Rockford. If you're passionate about helping patients regain their mobility and independence in a setting that feels like home, this role is for you. 📍 Position Details Location: Rockford and nearby communities (within 30 minutes preferred) Training: 2 weeks in Arlington Heights (tailored to your experience level) Visits: In-home therapy sessions, typically 45–60 minutes each Minimum of 5 visits per week Experience: Home Health experience strongly preferred 🚗 Work Location On the road — providing therapy where patients feel most comfortable: at home Ready to help patients move forward—literally and figuratively? Requirements
- For PTA: Valid Illinois PTA license (PTA is preferred, but PTs are also welcome)
- Experience in Home Health (preferred) or outpatient/skilled nursing settings
- Ability to work independently in diverse home environments
- Strong clinical reasoning and compassionate communication skills
- Reliable transportation and a valid driver’s license
- Illinois PT or PTA license (required)
- Valid Driver’s License (required)
Nice To Haves
- Experience in Home Health (preferred) or outpatient/skilled nursing settings
- CPR/BLS certification (preferred)
Responsibilities
- Conduct in-home evaluations and develop personalized treatment plans
- Provide hands-on therapy to improve mobility, strength, and balance
- Guide patients through exercises and functional activities
- Educate patients and caregivers on safe movement and home exercise programs
- Collaborate with RNs, OTs, and physicians for coordinated care
- Document progress and adjust plans to help patients reach their goals
