About The Position

RVNAhealth is seeking seeking licensed Physical Therapy Assistants (PTAs) to join our team, with opportunities available in both Home Health and In-Home Outpatient (IHOP) settings. We are actively hiring for our Northern Territory, which includes towns of New Milford, Newtown, Sandy Hook, Bethel, Brookfield, Danbury, and New Fairfield. We are hiring for full-time, part-time or per-diem opportunities to provide high-quality, one-on-one care to patients in their homes, supporting recovery, mobility, and independence. Position Options Home Health PTA (Medicare Part A) Work under the supervision of a licensed Physical Therapist to provide skilled, short-term therapy services to patients recovering from illness, injury, or hospitalization. In-Home Outpatient PTA (IHOP – Medicare Part B) Provide ongoing, outpatient-level therapy services in patients' homes and assisted living communities, often supporting longer-term rehabilitation and post-surgical recovery. What You'll Do (Both Roles) Deliver compassionate, high-quality care focused on patient outcomes Build strong relationships with patients and caregivers Manage a productive caseload and travel within assigned territory Utilize electronic documentation systems effectively Collaborate with therapists and interdisciplinary teams

Requirements

  • Associate degree from an accredited Physical Therapist Assistant program
  • Current Connecticut PTA license in good standing
  • Reliable vehicle and valid driver's license; ability to travel daily
  • Comfort with technology and electronic documentation

Nice To Haves

  • 2+ years of clinical experience preferred (home health and/or outpatient a plus)
  • Strong communication and teamwork skills

Responsibilities

  • Deliver therapeutic exercises and treatments based on the PT's plan of care
  • Provide hands-on care including gait training, strengthening, and mobility exercises
  • Assist patients with functional activities to improve independence and safety
  • Monitor and document patient progress; communicate updates to supervising PT
  • Participate in case conferences and interdisciplinary care planning
  • Maintain accurate, timely documentation in compliance with Medicare Part A requirements
  • Travel locally to patient homes within assigned territory
  • Implement treatment plans developed by the Physical Therapist
  • Support patients recovering from orthopedic procedures and chronic conditions
  • Provide progressive strengthening, balance training, and mobility interventions
  • Monitor patient response to treatment and communicate with supervising PT
  • Assist with adaptive equipment training and home exercise programs
  • Ensure accurate documentation and compliance with Medicare Part B guidelines
  • Deliver consistent, patient-centered care in a home-based setting
  • Deliver compassionate, high-quality care focused on patient outcomes
  • Build strong relationships with patients and caregivers
  • Manage a productive caseload and travel within assigned territory
