About The Position

This position is responsible for collaborating with MHO Providers and Reimagine Care to triage incoming clinical calls. The primary focus will be on handling triage calls for patients that are not on active treatment with chemotherapy or radiation. This position will also be responsible for medication refills when appropriate per MHO Standing orders. Controlled substance refill requests will be forwarded on to the ordering prescriber. Performs telephone assessment, developing a plan of care, and implementing interventions as appropriate. Serves as a patient advocate by coordinating care and facilitating communication between patient, providers, front desk, and centralized scheduling. There is a possibility for a remote work agreement.

Requirements

  • High school diploma or GED required.
  • State of Indiana Registered Nurse License required.
  • Proficient in Microsoft Office; experience with electronic medical records and medical software is a plus.
  • Excellent communication and organizational skills.
  • Ability to maintain professionalism in high-stress, fast-paced environments.
  • Strong interpersonal skills, capable of building rapport with patients, families, and colleagues.

Nice To Haves

  • Oncology experience preferred, with Chemo/ONS certification highly desirable.

Responsibilities

  • Assures compliance with all governing regulatory agencies and internal standards and requirements. Identifies and assists in resolving operational issues.
  • Independently performs telephone triage assessment, communication specific findings to the physician and developing a plan of care which correlates with the medical treatment plan. Utilizes standing orders as appropriate when developing an individual plan of care.
  • Responsible for assessing educational needs and providing information as needed. Continually updates knowledge base of community resources available.
  • Continually updates oncology knowledge base by participating in staff meetings, attending in-services as required in the RN job description.
  • Documents all telephone conversations, treatment plans, and evaluations to allow all staff members to intervene on the identified patient needs.
  • Consistently displays good clinical judgment and problem-solving skills with an ability to make independent decisions, utilizing oncology and hematology knowledge base and established MHO standing orders.
  • Serves as a resource for forwarding on any patient concerns related to insurance issues that they may have.
  • Performs within the registered nurse job description, upholding MHO policy and procedure always.
  • Maintains the practices and principals set forth by MHO leadership with a strong commitment to service, excellence, and quality.
  • Maintains professional behavior, confidentiality and discretion always, along with the ability to work with all levels of staff with a confident and professional demeanor.
