About The Position

Summary: Assist Physician, PA and/or NP to assess, plan, implement and evaluate the care needs of each patient and provide professional nursing care in the clinic setting. The telephone triage nurse assures that communication with each caller will be handled in a professional and tactful manner. When appropriate, the caller's symptoms will be assessed and triaged using the nursing process and approved guidelines in order to assist in obtaining the appropriate level of care.

Requirements

Graduate of accredited practical nurse program.
Must have active Illinois license and current BLS.
2 years experience in a clinic setting.

Responsibilities

Provide scope of care under basic nursing practices.
Greet patients and prepare them for provider examination. Screen patients for appropriate information, obtain medical history and take vital signs.
Provide adequate documentation and appropriate paperwork on patient record in proper charting order. Report abnormal test results to provider in timely manner and record required information on Abnormal Test Results Log.
Maintain and review patient record, chart and other pertinent information.
Participate in patient and family/caregiver education to support patients continued care including medication and treatment instructions.
Perform all in-house testing, including proficiency testing, and controls documenting results on appropriate quality control logs.
Assist in carrying out orders to schedule referrals and outpatient testing.
Triage patients on telephone following office protocol under supervision of provider.
Administer prescribed medications, injections and treatments in accordance with nursing standards documenting actions performed on appropriate logs and patient chart. Observe, record and report patient's condition and reaction to drugs and treatments to provider.
Respond promptly to each incoming call to the nurse line to ensure timely assessment of patient needs.
Assist with other duties as assigned, including referrals and follow-up calls
