Position Summary The Pharmacy Tech, Medication History is responsible for the medication history process for admitted patients. This position demonstrates efficacy in evaluating documented medication histories using structured patient interviews and standardized forms and processes. Furthermore, this position is responsible for ensuring medication lists are updated for all high-risk patients. Their role is designed to improve accuracy of prior admission medication histories and optimize patient outcomes through interdisciplinary medication management in an inpatient acute care hospital setting.

Requirements

Minimum three (3) years of experience in an inpatient hospital pharmacy or community pharmacy setting required
Current California Registered Pharmacy Technician license required

Nice To Haves

Graduated from a Pharmacy Technician Training Program preferred
Current Certified Pharmacy Technician (CPhT) certificate preferred

Responsibilities

Performs thorough patient medication histories and documents accurately into the electronic medical record.
Able to perform the duties and functions of a dispensing Pharmacy Technician.
Participates in quality assurance programs by maintaining medication history data and reporting any significant medication errors.
Demonstrates good customer services skills with colleagues and communicates effectively with other departments.
