About The Position
The Clinical Pharmacist is responsible for providing medication therapy management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient and cost-effective. Pharmacy services that we desire to incorporate into future clinical pharmacy practice: Patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various scholarly activities. The Pharmacist performs both clinical and distributive tasks as necessary. The Pharmacist will provide training and education to students through practical and didactic experiences. Incumbent acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Pharmacy Director. The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies. Provides pharmacy services for Hospital inpatients, Specialty/General Clinics, Nursing Home and work collaboratively with our Community Pharmacy team.
Requirements
- Pharmacy degree from an accredited college of pharmacy
- Licensed to practice in the State of Wisconsin
- Minimum one (1) year hospital experience
