Please follow this link for a closer look at what it’s like to work at Children’s Wisconsin: https://www.instagram.com/lifeatcw/ Job Summary Under the supervision of pharmacy residency leadership provides comprehensive pharmacy services that support working towards completing all required pharmacy residency activities as defined by the American Society of Hospital Pharmacy (ASHP) and the Children's Residency Manual. The pharmacy residency program at Children's Hospital of Wisconsin will provide the resident opportunity to develop clinical skills in a pediatric acute care hospital. The program will include rotations in, but not limited to: acute and ambulatory care settings, general pediatric medical/surgical areas, neonatal intensive care unit, pediatric intensive care unit, pediatric oncology and bone marrow transplant unit, pediatric drug information, pediatric pharmacokinetics service, pediatric investigational drug studies, pediatric solid organ transplant, pediatric infectious disease, and pediatric trauma and emergency response. The resident will assist in developing clinical programs and provide centralized and decentralized pharmacy staffing

Requirements

  • Terminal Degree (PhD, MD, etc.) Doctor of Pharmacy required
  • Competent in sterile and non-sterile compounding as defined by USP chapters 795, 797, and 800.
  • Analytical ability to troubleshoot and investigate prescription problems with providers.
  • Interpersonal skills to work with a variety of technical and professional staff.
  • Documentation skills necessary for residency and for pharmacist standard of practice
  • Ability to differentiate colors and visually inspect and confirm pharmacy technicians’ medication preparations.
  • License Pharmacist (40) - State of Wisconsin required
  • Provides care appropriate to patient population and as described in applicable policies and procedures.

Responsibilities

  • Reviews medication orders to ensure that safe and appropriate pharmaceutical care is provided. This includes clarification of orders, executing practice agreements, participating in interdisciplinary rounds, serving as drug information expert and resource for other disciplines, and documenting these activities accordingly
  • Provides care during medical emergencies including Rapid Response Team response, codes, and traumas.
  • Completes and reviews medication histories for all hospitalized patients.
  • Supervises the day to day activities of the pharmacy technicians. Provides the final verification for the accuracy, validity, completeness, and appropriateness of the medications prepared by a technician.
  • Works with pharmacist colleagues and pharmacy leadership in developing clinical programs, monitoring clinical activities of the department, addressing drug information questions, and providing pediatric pharmacy educational programs.
  • Prepares and dispenses all forms of medication for the inpatient population by interpreting physician orders and communicating appropriately with the medical team.
  • Ensures the security and appropriate documentation regarding medications. In particular, provides oversight in the handling of controlled substances to monitor for and guide others in proper disposal, storage, and recordkeeping.
  • Provides training and education to all types of learners within the pharmacy department.
  • Documents pharmaceutical care per hospital and department specific policies and procedures.
  • Participates in and completes required activities and projects for department and hospital committees as assigned to meet the requirements of the residency program outlined in the CW Pharmacy Residency Manual and in-line with ASHP residency standards.
