About The Position

The Siena Campus, a 326-bed acute care hospital opened in 2000, expanded the services of the Rose de Lima Campus to meet growing healthcare needs. The pharmacy practice resident (the resident) will actively participate in formulary management, medication management, and pharmacotherapy services. The resident will review, process and verify physician orders for appropriateness while ensuring prompt and accurate delivery of medications to patients. The resident will provide drug information to health care professionals and patients to assist in providing accurate medication therapy. The scope of services provided by the resident shall include accurate order entry, accessing allergy information, clinical interventions, medication dosing, renal dosing, hyperalimentation preparation, and medication counseling. The resident will assist with pharmacy staff development, nursing and prescriber education, and pharmacy student training (when applicable). The resident will maintain a cordial and professional work environment.

Requirements

  • Doctor Of Pharmacy Completion of a Post Graduate Year 1 (PGY1) pharmacy practice resident program., upon hire
  • Pharmacist: NV, upon hire
  • Basic Life Support - CPR, upon hire
  • Advanced Cardiac Life Support, within 60 Days

Responsibilities

  • Actively participate in formulary management, medication management, and pharmacotherapy services.
  • Review, process and verify physician orders for appropriateness while ensuring prompt and accurate delivery of medications to patients.
  • Provide drug information to health care professionals and patients to assist in providing accurate medication therapy.
  • Ensure accurate order entry, accessing allergy information, clinical interventions, medication dosing, renal dosing, hyperalimentation preparation, and medication counseling.
  • Assist with pharmacy staff development, nursing and prescriber education, and pharmacy student training (when applicable).
  • Maintain a cordial and professional work environment.
  • Ensures compliance with the hospital's formulary and approved policies and protocols related to medication management.
  • Supervises unit dose drug distribution program: Ensures the accurate and prompt delivery of unit dosed medications to patients.
  • Prepares intravenous products in accordance with written departmental guidelines.
  • Monitors patient medication therapy and evaluates initiation of approved protocols.
  • Assists nursing units/areas to monitor the proper storage and distribution of patient's floor stock, controlled substances and emergency medications.
  • Appropriately participates in the maintenance and management of patient's medication orders; in regards to order evaluation, order entry, storage and retrieval.
