The Pharmacy Clinical Specialist – Internal Medicine is based at Hillcrest Hospital, a Cleveland Clinic hospital that supports a large acute care facility with multiple inpatient units. The pharmacy department partners closely with hospitalist medicine teams, specialty services, and interdisciplinary care teams to deliver comprehensive medication management and clinical pharmacy services. Caregivers in this role collaborate with clinical pharmacists, pharmacy technicians, hospitalists, residents, nursing staff, and other members of the care team. This highly integrated role includes active participation in multidisciplinary rounds, medication management discussions, and clinical decision-making, as well as supporting resident education and working with other pharmacy specialists to optimize patient outcomes. This position offers the opportunity to practice at the top of your license within a collaborative environment known for clinical excellence, innovation, and professional development. Daily responsibilities include participating in hospitalist rounds, optimizing complex medication regimens for hospitalized patients, providing pharmacotherapy education to residents and learners, collaborating with physicians on medication-related decisions, supporting medication safety and stewardship initiatives, contributing to research and quality improvement efforts, and participating in staffing responsibilities approximately every ninth weekend. A caregiver in this position will work Monday through Friday from 7:00am to 3:30pm. Weekend expectations occur every 9th weekend. No holiday, on-call or floating requirements.

Requirements

  • PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy
  • If hired for a specialty area where specialty residency training is available, then specialty residency is required
  • Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire
  • Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring
  • For individuals required to administer immunizations: current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire
  • For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or American Red Cross or certification obtained within 1 year of hire

Responsibilities

  • Coordinate and provide pharmaceutical care for patients.
