Performs clinical reviews to all hospital services, meeting state, federal, professional and JCAHO standards. Coordinates these services with related departments, such as the Medical Staff, Patient Care Services, Quality Improvement, Infection Control, etc. Promotes the maintenance and expansion of the clinical programs. Participates in student clinical rotations from accredited colleges of pharmacy. Prepares and maintains various reporting programs, develops and maintains effective drug therapy optimization and cost reduction programs. Monitors policies and procedures and enforces the criteria. Evaluates medication usage, and is involved in Pharmacy and Therapeutics activities.

Requirements

  • Bachelor's or PharmD degree from an accredited college of Pharmacy.
  • Licensed to practice Pharmacy in New Jersey, by the NJ Board of Pharmacy.
  • Minimum of 3 years of hospital Pharmacy clinical practice.

  • Supervisory and preceptor experience preferred.

Responsibilities

  • Conducts drug usage evaluations on selected drugs, through an approved and established criteria and appropriate knowledge of the ages of patients served at PMC.
  • Perform antibiotic surveillance on formulary antibiotics, as well as non-formulary drugs, based upon the targeted products selected through the DUE process, as well as the needs of the patients served, including, but not limited to the ages of the patient population.
  • Monitors the culture and sensitivity reports in conjunction with the Microbiology Department of the hospital. Performs periodic reviews with the Chief of Infectious Disease and the consultants.
  • Monitors Adverse Drug Reaction (ADR`s) in accordance with the ADR Monitioring program. Reports the information to Clinical Manager, for inclusion in the ADR report, which is presented to the Pharmacy and Therapeutics Committee.
  • Coordinates and performs educational programs for the Pharmacy, Medical Staff, Patient Care and ancillary departments, as well as community outreach programs.
  • Supervises the preceptor program and coordinates the student activities in training from the affiliated colleges of pharmacy, including all orientation activities and documentation required by the colleges.
  • Assists in the maintenance of the clinical component of the information system, including training, report management and staff development. Works as Pharmacy liaison with the IS Pharmacy analyst.
  • Positive customer service skills including AIDET and 5/10.
  • Participates in the quality improvement program for the pharmacy, including compliance with the national patient safety goals.
  • Functions in a management capacity, in the absence of the director or assistant director, for administrative and personnel functions.
  • Participates in hospital committees, as assigned.
  • Functions in the retail pharmacy areas, as needed.
  • Operates the Omnicell system with efficiency and accuracy.
  • Other duties and/or projects as assigned.
