About The Position

The pharmacist works independently to manage all aspects of the medication distribution process to ensure that inpatients and ambulatory patients receive accurate and appropriate drug therapy. The pharmacist manages the daily operations of the inpatient pharmacy including sterile compounding, drug delivery, clinical services, and provides drug information to the healthcare team.

Requirements

Licensed as a registered pharmacist in the State of Oregon
One (1) year of experience as a pharmacist
Excellent customer service skills
Reads, writes and speaks conversational English
Deals with multiple demands in a fast-paced pharmacy in an accurate efficient, calm, and pleasant manner
Pharmacy computer skills desired, must be proficient at typing
Presents a professional appearance and appropriate interpersonal skills
Communicates effectively in verbal and written form with the healthcare team and with patients
Functions as a cooperative, flexible team member in a positive manner
Consistently utilizes problem-solving techniques to resolve complaints or concerns
Demonstrates knowledge of sterile compounding technique
Must be able to work independently with no direct supervision, set priorities, make critical decisions, respond quickly to needs, and adapt to changes in workload
BLS required within 90-days of hire
ACLS required within 90-days of hire

Nice To Haves

Completion of a PGY-1 pharmacy practice residency or eligible to take the board-certified pharmacotherapy specialist (BCPS) examination
Completion of a PGY-2 pharmacy residency or eligible to take a specialty board certification examination
Bilingual skills a plus
Previous experience with the EPIC electronic medical record and Omnicell

Responsibilities

Verifies orders and maintains patient profiles
Screens medication orders for clinical appropriateness, dosing, potential adverse events, drug interactions, and formulary appropriateness
Performs critical dosing calculations for all patients of all age groups (neonate, infant, pediatric, adult, and geriatric)
Appropriately documents in the patient chart decisions related to drug therapy
Doses and monitors select medications, with appropriate documentation
Provides drug information to other members of the healthcare team
Provides education counseling to patients
Participates in patient care rounds, as appropriate
Attends departmental staff meetings and other educational seminars
Supervises support personnel: educates and checks technician work ensuring accuracy and integrity of drug products
Triages and prioritizes workload based on patient status
Accurately dispenses medication in a timely fashion
Follows all regulations and policies
Responds and participates in emergency codes and rapid sequence intubation
In collaboration with pharmacy management, responsible for daily pharmacy operations
Participates and delivers hospital educational programs
Performs additional duties in the absence of the pharmacy management
Participates in medication utilization evaluations and quality improvement projects
Maintains perpetual inventory for all controlled substances
Fills prescriptions for patients being discharged
Participates in extemporaneous compounding, including sterile medications and chemotherapy according to USP 797 and USP 800
Orders medication and supplies from wholesaler
Maintains adequate supply of pharmaceuticals and related materials
Expectation to cover evening shift, night shift, weekends, and holidays per pharmacy procedure
Ability to cover discharge, oncology, and ambulatory (anticoagulation) shifts after completing the appropriate competency training
