About The Position

A pharmacist, pending licensure, shall be a recent graduate from an accredited School of Pharmacy who is waiting to take the Pennsylvania State Board of Pharmacy Examination; or a pharmacist who has been licensed to practice Pharmacy by another state and is applying for reciprocity. All medication orders reviewed and processed by the pharmacist, pending licensure, must be verified and approved by a licensed pharmacist prior to being dispensed to a patient. All medications must be approved by a licensed pharmacist prior to being dispensed to a patient.

Requirements

  • Doctorate Pharmacy Required
  • Pharmacokinetics, drug literature review, therapeutics, computer applications, aseptic technique. (Required proficiency)
  • Licensed Pharmacist (Pennsylvania) - Pennsylvania State Licensing Board - within 6 months - Required

Responsibilities

  • Reviews and interprets medication orders for inpatient orders for inpatients using the hospital information system (HIS) and the pharmacy computer system to ensure that the medication is appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state.
  • Ensures dose is appropriate for age, weight and disease state 100% of all medication orders.
  • Reviews orders for potential allergic reactions 100% of all medication orders.
  • Reviews for drug interactions 100% of all medication orders.
  • Evaluates for potential drug incompatibilities 100% of all medication orders.
  • Reviews for therapeutic duplication 100% of all medication orders.
  • Maintains patient medication profile in the pharmacy computer system to contain all the pertinent information necessary to safely dispense medications.
  • Access privileges to the PCSI-Pharmacy medication order processing system shall be limited so as to prevent the unsupervised approval of any medication orders in the production environment.
  • All orders entered into the production environment will be held in a pending approval state until approved by a licensed pharmacist. For the purposes of completing all Pharmacist training modules; unsupervised approval privileges will be granted in the test/practice environment of the application. All orders approved in the training environment will be reviewed by a licensed pharmacist but in no way will involve the approval of medication orders for actual patients.
  • Input or update allergy information 100% of all medication orders.
  • Input or update intervention information 100% of all medication orders.
  • Input of medication orders; understanding and using the computer system in a manner that insures the proper entry of drug orders, information, and patient charges 100% of time.
  • Prepares medication accurately in an appropriate container, labeled in an appropriate manner and stored at the recommended temperature. All medications prepared must be verified and approved by a licensed pharmacist prior to being sent to a patient.
  • Weighs, measures and mixes ingredients; compounds and prepares capsules, emulsions, liquids, suspensions, powders, ointments and intravenous solutions using appropriate equipment in a correct manner 100% of the time.
  • Prepares SVP, LVP, TPN and chemotherapeutic solutions as required using aseptic technique within a laminar air flow hood as per Aseptic Technique and Antineoplastic Policy and Procedures.
  • Prepares and stores medications in a manner that ensures compatibility and stability within the anticipated time of use. Reviews literature or conducts studies to determine stability and compatibility when necessary.
  • Completes scheduled department orientation/training modules. Reviews and approves simulated medication orders on test patients only.
