Under the direction of the Director of Pharmacy, the Ambulatory Clinical Pharmacist is responsible for developing, implementing and maintaining pharmacist-managed ambulatory care and/or specialty pharmacy services at St. Peter's Health Medical Group as part of a team-based patient care model. JOB DUTIES INCLUDE BUT ARE NOT LIMITED TO: - Develops and implements drug protocols and collaborative practice agreements - Monitors drug therapy according to established protocols and documents all patient care activities - Collaborates with providers in medication therapy management to provide services such as drug interaction checking, side effect management, medication counseling, and optimizing medication therapy. - Coordinates with scheduling and billing to optimize workflow, productivity, and reimbursement - Collaborates with the outpatient retail and specialty pharmacies for benefits investigation….. - Collaborates with designated staff to identify and pursues copayment assistance, patient assistance, or foundation assistance for patients Collaborates with members of the care team to optimize patient outcomes - Establishes quality measures, monitors and reports quality data, and identifies opportunities for quality improvement - Develops educational programs for patients, providers, and staff - Effectively markets ambulatory care and specialty pharmacy services to patients, providers, and staff - Participates on key clinical and operational committees - Assures compliance with clinical best practice standards and guidelines, accrediting agencies, billing guidelines, and all state and federal regulations. - Precepts pharmacy residents and APPE students - Responsible for oversight of the outpatient antimicrobial stewardship program in applicable clinic settings - Performs other pharmacist duties as assigned
