The Ambulatory Clinical Pharmacist provides comprehensive pharmaceutical care for patients in the ambulatory setting. This consists of ensuring drug therapy that results in safe, effective, and cost-conscious use of pharmaceuticals. In addition, the pharmacist ensures patients have appropriate access to prescribed medications and are provided with medication education as well as adherence tools when needed/appropriate. Candidates must demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities. This pharmacist will work in the Primary Care clinics to serve patients with various disease dates. They will be responsible for conducting referral-based pharmacist-led comprehensive medication management appointments for the referral reasons such as Diabetes, Hypertension, Dyslipidemia, GLP-1 for Obesity, Comprehensive Medication Review , Deprescribing, Medication Tapering Plan and Device Education.
