About The Position

Stormont Vail Health PGY2 Pharmacy Residency Program provides comprehensive pharmaceutical care to patients through optimizing medication therapy and promotes health, wellness and disease state prevention under the guidance of Clinical Pharmacy Specialists. SVH PGY2 Pharmacy Residents will uphold American Health-Systems Pharmacists and American College of Clinical Pharmacist philosophy of pharmaceutical care; which is blending a caring orientation with specialized therapeutic knowledge, experience and judgment for the purpose of ensuring optimal patient outcomes. Stormont Vail Health PGY2 Pharmacy residency program will abide by the American Health-Systems Pharmacists residency accreditation requirements. SVH PGY2 Emergency Medicine Pharmacy Residents will apply evidenced based therapeutic guidelines, emerging technologies and relevant legal, ethical, social, cultural, economic and professional principles. SVH PGY2 Emergency Medicine Pharmacy Residents will assume responsibility and accountability for managing medication therapy in direct patient care settings, whether practicing independently or in consultation/collaboration with other healthcare professionals. SVH PGY2 Emergency Medicine Pharmacy Residents will generate, disseminate and apply new knowledge that contributes to improved health and quality of life. SVH PGY2 Emergency Medicine Pharmacy Residents will become experts in the therapeutic use of medications and routinely provide medication therapy evaluations and recommendations to patients and healthcare professionals. SVH Pharmacy Residents will provide clinically valid information and advice regarding the safe, appropriate and cost-effective use of medications. SVH PGY2 Emergency Medicine Pharmacy Residents will gain experience in various pharmaceutical care patient services including but not limited to the following; critical care, NICU, emergency, pediatrics, trauma, and internal medicine.

Requirements

Bachelor's Degree Science in Pharmacy. Required or Doctorate Pharm D from an accredited school of pharmacy and completion of an accredited PGY1 residency program. Required
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from residency directors, preceptors, other healthcare professionals, patients, and the general public.
Excellent customer service skills. (Required proficiency)
Ability to apply advanced pharmaceutical mathematical concepts including but not limited to the following: pharmacokinetic calculations, Child Pugh score, Cockcroft- Gault calculations, and other pertinent calculations based on patient population. (Required proficiency)
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. (Required proficiency)
Spreadsheets, Internet, E-mail, PowerPoint, Word, Excel and database software. (Required proficiency)
Eligible for licensure in the state of Kansas. Must obtain active license within 60 days of employment. Failure to obtain active licensure within designated time frame will result in termination.

Responsibilities

Discuss medication order clarification with the prescriber, document any changes in inpatient and pharmacy records, and inform other staff of medication order changes.
Promote the use of formulary by converting non-formulary orders to formulary if possible, but coordinates procurement of non-formulary drugs if determined medically necessary.
Review medication orders for accuracy and appropriateness.
Perform order verification for assigned areas.
Coordinate with technician to resolve drug distribution related issues.
Coordinate with technician to resolve Omnicell related issues (i.e. discrepancies, access issues, etc).
Follow and coordinate with technician to assure patient care units comply with JCAHO medication standards.
Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective clinical pathways.
Develop and conduct target drug programs and medication use evaluations as needed, and report results to the Pharmacy and Therapeutics Committee or subcommittees.
Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.
Promote and demonstrate rational, cost-effective drug therapy.
Monitor the use of expensive medications to assure there is use consistent with approved criteria (e.g. alteplase, snake antivenin, eptifibatide, surfactants, etc.).
Provide comprehensive pharmaceutical care to designated patient care department.
Participate in daily patient care activities including review of labs, microbiology, vital signs, and MAR to assure medication therapy is appropriate or if additional intervention is required.
Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Provide pharmacokinetic consultations and other dosing as required.
Participate in drug therapy management of medical emergencies (code blue, trauma, stroke, snake bite etc.)
Counsel/triage patients on discharge/transfer medications as necessary.
Prevent, detect, monitor, document and report adverse drug reactions and medication errors.
Expedite care for Core Measure patients to meet benchmarks.
Assess insurance drug coverage in advance of discharge/transfer and assist in preauthorization for medications if necessary.
Provide presentations, publication and other informative activities pertaining to designated patient care area and other drug-related topics to the health care community and general public.
Active participant in SVH Antibiotic Stewardship Program which includes appropriate empiric therapy, monitoring appropriate antibiotic therapy based on cultures and sensitivities, recommending de-escalation antibiotic regimen and appropriate duration.
Recommend appropriate pain management therapy and follow patient to assure pain regimen is appropriate.
Required to complete all ASHP PGY2 Residency Accreditation standards.
Participate in facilitating professional development for other pharmacists.
Teach Pharmacy Students and PGY1 Pharmacy residents: • Teach PharmD and IPPE/APPE students • Teach PGY1 residents • Help develop and maintain rotation objectives, student expectations and evaluations
Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
