About The Position

Frederick Living is a community focused on purpose, opportunity, and living life to its fullest. As a Personal Care – LPN, you will be responsible for the total nursing care program in Personal Care through nursing personnel management and direction. This role involves playing a key leadership role in ensuring residents receive high-quality, compassionate care every day. You will oversee daily nursing care operations in Personal Care, working closely with your team and the Clinical Manager to deliver excellent resident care.

Requirements

  • Graduate of an accredited Licensed Practical Nursing School with current licensure by examination in the Commonwealth of Pennsylvania
  • Good physical and mental health — must provide a satisfactory physician exam and TB Test or chest X-ray upon hire and as required thereafter
  • Strong communication skills — must be able to understand, read, write, and speak English effectively
  • Ability to grasp, kneel, pull, push, reach, stand, and walk long distances to perform essential job functions
  • Ability to lift weight in excess of 50 pounds
  • Ability to feel, hear, see, smell, and speak adequately to perform essential job functions
  • Ability to work in inside environmental conditions
  • Willingness to work every other weekend and holidays as needed
  • Willingness to assist in emergencies as needed

Nice To Haves

  • Prior experience in geriatric nursing preferred

Responsibilities

  • Act consistently as a team player and foster a collaborative environment
  • Make out daily assignments for resident care and assure completeness by end of shift
  • Make daily rounds of all residents and areas, recognizing, interpreting, and reporting conditions to the Clinical Manager
  • Make rounds with physicians and call as needed
  • Administer medications, oral and IM, as assigned daily
  • Perform treatments as ordered and routine tasks such as BPs, weights, and intake/output
  • Order medications, diets, supplies, and equipment as needed
  • Complete admission and discharge assessments on all residents as necessary
  • Conduct team conferences at daily report as needed
  • Check equipment for cleanliness and condition, including med room, linen storage, and central bathing areas
  • Teach and instruct nurse aides whenever necessary
  • Assist in hands-on resident care and answer call bells as needed
  • Monitor lab testing requests/results and call or fax abnormal readings to physicians
  • Report all incidents and hospitalizations to Personal Care Managers immediately
  • Maintain emergency equipment and supplies
  • Enter demographics and medications into the computer for admissions and medication changes
  • Assess residents when ill or in pain and take immediate steps to resolve
  • Update families when there is a change in resident status
  • Make rounds two times per shift and PRN in Aspen
  • Schedule coverage and cover call-offs
  • Communicate resident needs and care levels to outside agencies
  • Monitor and complete wound care
  • Train new employees, nurses, and unit clerks
  • Support resident physicians in the medical suite
  • Conduct audits of medication carts and resident records
  • Abide by the organization's code of conduct at all times

  • Provide excellent customer service to residents, staff, volunteers, families, visitors, and vendors
  • Maintain resident and staff confidentiality and support person-centered care initiatives
  • Perform other related duties as required
