Requirements

Graduate of an approved LPN program (RN candidates also considered).
Current, unencumbered LPN license in Pennsylvania.
Minimum 2 years of nursing experience

Nice To Haves

at least 1 year in gerontology or long-term care preferred.

Responsibilities

Lead and supervise the Neighborhood Care Team to deliver quality personal and clinical care to residents.
Administer medications and treatments accurately and in accordance with physician orders and facility policies.
Assess and monitor residents’ health status; promptly report changes and collaborate with physicians and families as needed.
Coordinate and delegate daily assignments to care for team members, ensuring efficiency and resident satisfaction.
Foster teamwork and mentorship, providing guidance and support to CNAs, PCAs, and other team members.
Promote a person-centered approach, ensuring dignity, choice, and respect for each resident’s needs and preferences.
Ensure compliance with all federal, state, and local regulations, as well as HIPAA Privacy and Security standards.
Collaborate with interdisciplinary team members to develop, implement, and evaluate individualized care plans.
Work a flexible schedule, including weekends and holidays as needed.
Assume accountability for compliance with federal, state and local regulations within the assigned duties and within his or her span of control
Will assume responsibility to ensure secure handling of information, and will adhere to HIPAA Privacy and Security standards
Perform additional duties as assigned by leadership
