The Personal Care Home Administrator (PCHA) is responsible for coordinating daily assignments of staff in the Community Residence, leisure groups, day program, home activities, and other program assignments as necessary. Regularly monitors employees and persons served in program areas. Responsible for maintaining open communications with all Beechwood employees across the transdisciplinary team. Responsible for communicating and/or conferring with residential managers, vocational, facilities management, and clinical employees as necessary.

Requirements

  • Associate’s degree (or 60 credit hours) and at least 2 years’ experience working with individuals with Traumatic Brain Injury.
  • Must be able to lift a minimum of 50 pounds.
  • Must possess certification as Personal Care Home Administrator in accordance with 55 Pa. Code 2600 licensing requirement within 6 months of employment.
  • Must successfully complete and pass the DHS competency test in accordance with 55 Pa. Code 2600 regulations.

Responsibilities

  • Implements the cognitive/physical strategies and supports that each person served needs to live a fulfilling and productive daily life.
  • Ensures that the daily program of the person served is consistent with their goals and maximizes participation in home and community activities.
  • Maintains professional/ethical relationships with persons served by parents/relatives and agencies.
  • Plans and implements Community residence activities and supports those served to ensure attendance at program functions including off and on-grounds medical appointments.
  • Responsible for the implementation of home goals and objectives which are consistent with the overall goals and objectives of the facility.
  • Collaborates with Health & Wellness staff on any and all physical health issues of persons served as soon as they become evident.
  • Participates on assigned committees as needed and attends assigned PRC, ITC, RSP, and other scheduled meetings as directed.
  • Responsible for on-the-job training of new Community Residence LSTs and on-going monitoring of performance by having a presence on all shifts.
  • Responsible for training all assigned employees in EHR and ensuring appropriate documentation is being entered.
  • Responsible for frequent review of EHR for accuracy and submission of daily census along with all other pertinent documentation.
  • Responsible for ensuring that data collection is in compliance with the Community Residence procedures.
  • Observes and ensures Community Residence is clean. Reports all maintenance needs, orders supplies, and maintains an inventory of persons served.
  • Ensures that all residential LST/pantry/housekeeping employees adhere to the policies and procedures of the facility and that the Community Residence is in compliance with all regulations/standards governing Woods Services.
  • Must be able to lift, use and have knowledge of adaptive equipment and positioning techniques as needed for the health, safety and well-being of those served.
  • Schedules, conducts, and documents LST group meetings and individualized supervision for all shifts
  • Schedules conducts and documents Community Residence meetings with persons served for the purpose of assuring a positive home environment where everyone is working together to assure good interpersonal relations and to maximize participation in home and community activities.
  • Assists direct supervisor in the completion of annual staff evaluations
  • Responsible for reporting to the direct supervisor or designee any issues arising from any complaint of improper conduct and address as directed.
  • Responsible for reporting to the direct supervisor or designee any unusual incidents involving a person being served, LST, or family.
  • Monitors petty cash and assists persons served with managing their personal finances, creating budgets, and being able to access their bank accounts.
  • Responsible to serve as Administrator under the DHS 2600 regulations and ensuring compliance with the standards which govern the Community Residences.
  • Plans and disseminates Community Residence LST schedules adequate to meet the needs of persons served and ensures minimum staffing ratio requirements including collaboration with direct supervisor or designee to ensure adequate coverage for all shifts
  • Works a flexible 40-hour schedule consistently, including every other weekend, and be able to work extended hours to cover necessary shifts and ensure visibility across shifts
  • Responsible for the completion and documentation of monthly Fire Drills and Water Temperature checks
  • Acts as Beechwood charge person as designated.
  • Demonstrates knowledge of acquired brain injury and is able to train staff on the most commonly used brain injury strategies and supports.
  • Is knowledgeable of the brain injury long-term supports and services, and how to use them to assure maximum participation in the home and community for each person served.
  • Demonstrates knowledge of DHS 2600 Regulations licensure standards and CARF brain injury accreditation standards and develops systems to ensure compliance in all areas.
  • Demonstrate effective communication skills both written and verbal to assure effective communication between residents, staff, and families. Demonstrate judgment to communicate significant information to upper management as needed.
  • Demonstrate ability to conform to all documentation such as goal writing, support planning, Rehab Service Plans, and family contacts.
  • Demonstrates skills necessary to hire, supervise, counsel, and discipline staff as needed.
  • Has leadership ability to manage crisis situations in the Community Residence and while functioning in the role of Beechwood Charge.
  • Demonstrates ability to work as part of a transdisciplinary team to facilitate treatment and care for persons served.
  • Knowledgeable of Woods Policies and Procedures and applies these consistently and fairly to all employees.
  • Demonstrates skills and systems to assure that all assigned Community Residences provide a high-quality environment for all Members to maximally participate in their homes and community.
