About The Position

The Person-Centered Planning Specialist plays an integral role in helping adults with intellectual and developmental disabilities plan their own lives and secure services. Success in this position will result in robust person-centered plans (PCPs) that value the person as the lead decision-maker and meet the timelines and criteria of the Jubilee PCP process.

Requirements

  • Bachelor's degree
  • Minimum of 3 years of professional experience delivering community-based services to people with intellectual and developmental disabilities.
  • Willingness to work occasional evenings.
  • Advanced written and verbal communication, with strong interpersonal skills. Use of person-first language and thinking is essential.
  • Excellent organizational/time-management skills and attention to detail to effectively manage documents and project lists.
  • Strong technology skills to effectively use Outlook, Word, Excel, Teams, Zoom, and data systems relevant to the position.
  • Desire to work with and learn from people who have intellectual and developmental disabilities required.
  • Demonstrated ability to use good judgment, take initiative, and make recommendations in resolving problems.

Responsibilities

  • Lead the annual PCP process for approximately 50 adults with intellectual and developmental disabilities who receive Personal Supports, Supported Living, or Group Home services from Jubilee.
  • Interviewing each person Jubilee supports using the Personal Outcome Measures (POM) tool, which covers 21 life indicators and up to 300 questions.
  • Coordinating and preparing materials needed for the POM session and PCP meeting.
  • Assessing the specific focus areas from the interview and creating a report with recommendations in collaboration with the person receiving services.
  • Monitoring and following up with the person receiving services and their support staff to ensure follow-through on recommendations.
  • Preparing reports based on Jubilee and national POM data.
  • Scheduling and coordinating the annual PCP meeting with the Coordinator of Community Services (CCS) and Jubilee’s Program Services team.
  • Writing and ensuring that all annual goals in the PCP are specific, measurable, and meet SMART goal criteria.
  • Writing client narratives to justify and apply for Maryland state benefits.
  • Completing authorization forms as specified by the Maryland Developmental Disabilities Administration (DDA) for each person.
  • Ensuring the annual review, facesheet, medical assessment, signature pages, updates, and action steps are completed and recorded for each meeting.
  • Monitor plans and follow up with the CCS’ to ensure timely dissemination of final PCPs and related documents in DDA’s Long Term Care Services and Supports (LTSS) system.
  • Enter the final PCP goals, behavior goals, tracking charts, and other documents relevant to each person’s services in iCare, a database used by Jubilee staff.
  • Coordinate mid-year PCP revisions as needed in collaboration with the CCS, Program Manager, and PCP team.
  • Ensure eligible Jubilee residential clients on your caseload are enrolled and maintain enrollment in the Maryland Medicaid Community Pathways Waiver.
  • Attend Medicaid Waiver meetings and apply and/or help families in their applications to obtain Medicaid benefits from DDA; this entails filling out information and gathering supporting documents for submission.
  • Submit the Medicaid Waiver Redetermination packet and/or assist families. Track, monitor, and follow up until a decision is made.
  • Keep record of all communication logs regarding follow-up and fill out an appeal form online if the decision is in contention.
  • Work collaboratively with the Quality Enhancement team and other departments to share and seek information as needed.
  • Complete other duties as assigned.
