About The Position

We are seeking a Permanency Coordinator to join our team at our program in Allentown, PA. The Permanency Coordinator works through the Statewide Adoption and Permanency Network (SWAN) to deliver SWAN units of service to including Child Profile, Child Preparation, Child Specific Recruitment, Family Profile, Placement, Finalization and Post Permanency Services (Assessment, Advocacy/Case Management, Respite, Support Group) in an effort to support permanency for children. The Permanency Coordinator will complete all assigned units of service according to established benchmarks and timeframes in order to promote timely permanency. Excellent writing, communication, organizational, and computer skills required. Travel and some evening hours necessary. Valid driver’s license and use of personal automobile required. Work Location: Office environment and community setting/resource home/residential facility/group home depending on youth location.

Requirements

  • High School Diploma or equivalent
  • One (1) year experience working in the child welfare or closely related system.
  • Excellent writing, communication, organizational, and computer skills required. Travel and some evening hours necessary. Valid driver’s license and use of personal automobile required.

Responsibilities

  • Knowledge of Department of Public Welfare regulations, SWAN Bulletins, and other requirements.
  • Completion of all units of SWAN service as assigned.
  • Attendance at SWAN and Merakey meetings as determined by program leadership.
  • Participation in ongoing training as indicated by State regulations and/or Merakey policy.
  • Support FCP staff in case planning activities to promote permanency for youth.
  • Market permanency services to county agencies and general public.
  • Promote the visibility of Merakey via local speaking engagements, etc.
  • Other duties as assigned.
