About The Position

Community of Hope is seeking a Perinatal Group Health Educator to provide group education focused on perinatal health, childbirth, infant care, and parenting to patients participating in group care. This position is located at our Family Health & Birth Center in NE, Washington, DC.

Requirements

  • High School Diploma/GED.
  • Valid BLS Certificate.
  • Ability to accommodate a non-traditional schedule to include regular evenings and weekends.
  • Ability to travel between COH locations. Ability to work at various work locations as groups will take place at three different sites or wherever is convenient for patients.
  • Strong interpersonal skills.
  • Experience providing health education in a group of individual setting.
  • Ability to express enthusiasm about breastfeeding and a desire to help birthing people and families enjoy a positive experience.
  • Knowledge of the CenteringPregnancy and/or CenteringParenting model (preferred) or experience working with perinatal groups.
  • Commitment and ability to work with underserved populations.
  • Proof of vaccinations is required. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.

Nice To Haves

  • Bachelor's degree in human services, public/community health, health education or social work.
  • Minimum of 2 years of experience in a health-related field strongly.
  • Ability to fluently speak Spanish or Amharic strongly.
  • Experience coordinating programs or initiatives and/or facilitating groups.

Responsibilities

  • Co-facilitates with providers multiple prenatal and well-child group appointments using the CenteringPregnancy/CenteringParenting model, incorporating the three major components: assessment, education, and support.
  • Facilitates and assists with the delivery of perinatal and newborn education to support positive health outcomes (i.e. breastfeeding and lactation, childbirth, and newborn care).
  • Administers surveys to prenatal and well-child groups and submits data to supervisor for purposes of evaluation of the CenteringPregnancy/CenteringParenting model.
  • Utilizes quality management reports to plan and conduct meaningful outreach to all eligible perinatal patients and caregivers of pediatric patients less than age 1. A variety of strategies for outreach are to be employed to include telephonic, in-person, and electronic.
  • Connects with prenatal patients (by 16-20 weeks of pregnancy) and parents of pediatric patients (by 6 weeks postpartum) to transition from individual medical care to group medical care.
  • Contacts patients regarding missed group appointments to ensure their continued engagement in group medical care and to close an gaps in care created by missed group appointments. Assesses barriers to adherence to group schedule.
  • Facilitates monthly group offerings that develop peer connection and support between families at COH using identified interventions (i.e. Parent Café, play groups, etc.).
  • Ensures meeting space for groups is set up to support group activities (materials, seating, play areas, etc.). Assesses group supply inventory (to include incentives, activity materials, cleaning supplies, snacks, etc.) prior to the start of all groups and provides notice of needed supplies to supervisor no less than 2 weeks prior to group starts.
