As a Per Diem Home Health Medical Social Worker, you will play a vital role in providing care to our patients in various areas of San Diego County. You will assess and evaluate patients' needs for community resources and in-home support services, assist in implementing the plan of care, support patients with advance directives, and maintain accurate clinical notes. Our electronic medical records system allows you to complete all charting inside the patient's home efficiently. Requirements

  • Master's Degree from a program accredited by the Council on Social Work Education.
  • Certified/licensed and in good standing within the state.
  • Minimum of one year of experience as a Medical Social Worker, preferably in a healthcare setting.
  • Good physical stamina and mental health.
  • Basic Life Support Certification (BLS) required.
  • Excellent communication skills and ability to follow written instructions.
  • Self-directed and able to work with little direct supervision.
  • Demonstrates tact, patience, and good personal hygiene.
  • Valid driver's license and insured automobile.

Nice To Haves

  • Experience in home health is preferred.

Responsibilities

  • Assessing and evaluating patients for community resources and in-home support services.
  • Assisting in the implementation of the plan of care.
  • Supporting patients with advance directives.
  • Preparing and submitting accurate clinical notes.
  • Completing all charting inside the patient's home using our electronic medical records system.
  • Collaborating with the interdisciplinary team to provide comprehensive care.
  • Participating in discharge planning and in-service education programs.
