The Hospice Social Worker (MSW) is a full-time, exempt (salaried) position responsible for providing advanced professional psychosocial support services to hospice patients and their families. This role addresses emotional, social, financial, and environmental needs to enhance quality of life during end-of-life care. As an exempt position, the Hospice Social Worker is expected to meet job responsibilities and patient care needs beyond scheduled hours when required to ensure continuity and quality of care.

  • Conduct comprehensive psychosocial assessments of hospice patients and families.
  • Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary hospice team.
  • Provide counseling, emotional support, crisis intervention, and anticipatory grief support.
  • Assist patients and families with advance care planning, including advance directives, goals of care discussions, and end-of-life decision-making.
  • Identify, coordinate, and facilitate access to community resources and financial assistance programs.
  • Educate patients and families on hospice philosophy, disease progression, coping strategies, and caregiver support.
  • Participate actively in interdisciplinary team meetings and contribute clinical insight to care coordination.
  • Maintain timely, accurate, and compliant documentation in the electronic medical record.
  • Support care transitions, discharge planning, and coordination with community providers as appropriate.
  • Collaborate with physicians, nurses, chaplains, volunteers, and other team members to provide holistic, patient-centered care.
  • Provide bereavement support and referrals in accordance with hospice regulations and VNA policies.
  • Ensure compliance with Medicare Hospice Conditions of Participation, state regulations, accreditation standards, and VNA policies.
  • Maintain strict patient confidentiality and compliance with HIPAA regulations.
  • Master's degree in social work (MSW) from an accredited program.
  • Current, active Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Kansas, or ability to obtain.
  • Minimum of one (1) year of experience in healthcare, hospice, home health, or a related clinical setting.
  • Valid driver's license with reliable transportation.
  • Ability to travel to patient homes, facilities, and community settings.
  • Advanced clinical assessment and counseling skills.
  • Strong interpersonal, communication, and documentation skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Compassionate, ethical, and professional approach to end-of-life care.
  • Strong organizational and time management skills.
  • Ability to manage emotionally complex situations with professionalism and empathy.
  • Proficiency with electronic medical records and basic computer applications.
  • Prior hospice or end-of-life care experience.
  • Knowledge of Medicare hospice regulations and community resources.
  • Experience working within an interdisciplinary healthcare team.
