About The Position

The Peer Support Specialist provides supportive services with a rehabilitation and recovery focus. Services are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles including hope, self-efficacy, and community living skills. The Peer Support Specialist is also responsible for monitoring and ensuring routine assessments are completed per contract requirements.

Requirements

  • High School Diploma or GED.
  • Have personal experience as a recipient of mental health services and/or substance abuse services with a willingness to share personal and practical experience, knowledge, and first-hand insight to benefit clients.
  • Demonstrated ability to maintain personal wellness and utilize recovery tools.
  • Certified by the Office of Mental Health (OMH) as a Certified Peer Specialist or Office of Alcoholism and Substance Abuse (OASAS) as a Certified Peer Recovery Advocate, or willing to become certified within 6 months of hire. Liberty Resources will cover the cost of CRPA training.
  • Ability to work collaboratively with a multidisciplinary team. Ability to effectively use an electronic health record.
  • Excellent communication skills
  • Strong organizational, engagement, and interpersonal skills.
  • Valid NYS driver’s license and access to reliable transportation required.

Responsibilities

  • Drawing on knowledge and skills acquired from personal experience with the mental health, medical, and substance use systems, provide one-to-one supportive services to individuals both on-site and in the community as outlined within the APS Code of Ethics and/or the NYCB Code of Ethical Conduct.
  • Provide support during Same Day Access (SDA) hours by engaging with clients upon arrival, reviewing their needs and PRAPARE screening responses, identifying potential resources, and offering peer-based support in alignment with the SDA workflow.
  • Conduct outreach and community-based visits to support engagement, promote access to care, and reduce service barriers for clients.
  • Coordinate and collaborate with the organization’s care managers, patient navigators, and clinical teams to ensure a seamless service experience.
  • Act as a positive role model for individuals by demonstrating a high level of personal wellness and recovery.
  • Complete necessary documentation including assessments, progress notes, and care plans as applicable and in accordance with program and agency standards

  • Assist individuals in identifying and utilizing personal recovery tools, such as relapse prevention plans, wellness activities, and self-help resources.
  • Provide recovery education and coaching to individuals and their families, emphasizing areas such as community involvement, wellness, spirituality, employment, and personal empowerment.
  • Use a personal vehicle for community-based appointments as applicable.
  • Respond flexibly to emergency or unplanned situations, which may involve schedule or location changes.
