Provides peer support to youth ages infant to 18 years old who may be struggling with mental health/behavioral issues at home, school, or in the community. This position facilitates contact with providers, assists with case management, care coordination, 1:1 peer counseling, and addresses issues that may be barriers to accessing treatment and/ or support in the community.

Requirements

  • 1 year of related experience working with youth
  • Lived experience that enhances relatability to clients being served
  • HS diploma or equivalent
  • Peer Support Specialist certification is required upon start, and certification as a traditional health care worker through the Oregon Health Authority
  • Fluency in English, both spoken and written
  • Intermediate mastery of business applications and productivity suites
  • Demonstrates resilience, awareness, and self-regulation in navigating challenges, conflicts, and obstacles/barriers.
  • Thrives in a diverse and multicultural environment; works with cross-functional teams to develop and implement strategies to enhance end-user experiences throughout the service lifecycle
  • Demonstrates exceptional commitment to serving and advocating for diverse and marginalized populations
  • Travels between worksites regularly; responsible for transporting clients in personal vehicle, which requires a valid driver’s license, auto insurance, and motor vehicle records clearance
  • Works in a variety of settings, including and not limited to LCSNW office locations, partner agency and community-based settings, client home/community settings, outdoor settings in varying weather conditions, and faith-based settings such as churches
  • Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program

Responsibilities

  • Meet regularly with clients, by providing coaching and mentoring, and model healthy and empowered behavior
  • Conduct outreach, education, and community engagement activities
  • Communicate with the treatment team about barriers and strengths
  • Acts as a positive role model for individuals being served in exploring their own path to wellness
  • Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of youth and their families
  • Provide case management and care coordination
  • Document services using electronic health records in a timely and accurate manner
